PRODUCT SUBMISSION FORMS (Excel Doc)
The submission form will open in Excel. For more than 4 submissions at one time, you may Copy the master worksheet onto other worksheets before filling it out, or simply start a new file. Fill out one form per product request and
Save As document with your company name Or Individual Last name included in file name.
Either attach the one document to an email to us,
or print each page to FAX or mail us.
CONFIDENTIALITY AGREEMENT (in same Excel Doc)
Fill out the Confidentiality statement signature block by typing your name and/or company name in the appropriate line, if emailing. Otherwise, include your hand signature.
